-Know the name and professional grade of the treating physician and how to contact him or her.
-For complete information about your illness and treatment.
-Learn about your medication, side effects and diet.
-Privacy and confidentiality during examination or treatment.
-Confidentiality of your medical record and the right of approving or refusing to release it (the approval is mandatory to receive treatment under insurance companies).
-Receive counseling regarding the estimated or absolute cost of your treatment and the relevant details.
-A second opinion about your diagnosis and treatment plan.
-Refuse a recommended treatment to the extent permitted by law and being informed about the medical consequences of the refusal.
-When transfer to another facility is required, you should be provided with complete explanation and alternatives.
-Ability to lodge a complaint without affecting negatively the quality of care provided.
-Protection from verbal abuse by any employee.

-Provide accurate and complete information concerning present complaints, past medical history, medication and other matters related to your health.
-Follow the treatment plan advised by the physician, including the instructions of nurses and other health professionals.
-Understand the course of the medical treatment and seek clarification if required.
-Treat all staff and patients with dignity and respect and not conduct any activity that will disrupt the workflow.
-Respect the privacy of others.
-Not bring alcohol, unauthorized drugs, weapons or pets.
-Comply with the No-smoking regulation.
-Take full responsibility for your belongings.
-Take responsibility for refusing treatment or advice.
